Dr. Arif is in the office on Mondays from 8 am to 4:00 PM, Wednesdays from 1 PM to 4:30 PM, and on Fridays from 8 AM to 1 PM.
Dr. Arif does Procedures at Medical City Frisco on Tuesdays and Thursdays. Dr. Arif does Procedures at the Endoscopy Center of Plano on Wednesdays.

Please attach a copy of the front and back of your insurance card. This is a requirement for new patients. If you are a follow-up patient, please attach an updated insurance card if applicable.

If you have an HMO plan, we will need a referral from your primary physician before your appointment can be scheduled. Please contact your Primary physician's office to make sure that they have sent us a referral.
